Assumption of Risk

I, the undersigned participant (or parent/guardian if under 18), understand that participation in soccer and related activities with Roots United FC involves inherent risks, including but not limited to injury, serious bodily harm, or death.

I voluntarily agree to participate and assume all risks associated with training sessions, games, practices, travel, and related activities organized or facilitated by Roots United FC.

Release of Liability

I hereby release, waive, and discharge Roots United FC, its owner(s), coaches, staff, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising from injury, damage, or loss that may occur as a result of participation in any Roots United FC activity.

Participant Agreements

I agree and acknowledge that:

I am physically fit and able to participate in soccer training and related activities.

I will follow all rules, instructions, and directives given by coaches and staff.

Roots United FC is not responsible for lost or stolen property at any training or game location.

I understand this is a recreational youth soccer program and that injuries may occur in the course of normal participation.

📷 Media Release

I grant permission to Roots United FC to use photographs and/or videos of me (or my child) for promotional purposes, including but not limited to social media, website, and printed materials.

🏥 Medical Authorization

In case of a medical emergency, I authorize Roots United FC and/or its representatives to seek and obtain emergency medical treatment for the participant if a parent or guardian is unable to be reached in a timely manner.

I HAVE READ AND FULLY UNDERSTAND THIS AGREEMENT AND SIGN IT VOLUNTARILY. I ACKNOWLEDGE THAT BY SIGNING, I AM GIVING UP CERTAIN LEGAL RIGHTS.
